As a Homecare Specialist, you will:
-Develop and maintain contact with healthcare professionals (e.g., physicians, discharge planners, social workers, healthcare coordinators) to provide updated information on Gentiva’s home health program offerings, guidelines, and policies.
-Ensure effective communication and collaboration with branch staff and field sales resources via weekly meeting and strong communications skills with the group.
-Conduct pre-discharge hospital visits at the physicians’ request to determine need and eligibility for homecare.
