Dialysis LPN/LVN
Description
This position will provide direct patient care for assigned patient(s). Perform all technical aspects of dialysis procedures for assigned patients as prescribed. Administer and monitor treatments in accordance with current treatment plan or as directed by the supervisor. Monitor and document dialysis treatment parameters on dialysis flow sheets. Document other information related to the care of the patient in the individual patient record. Responsible for reporting any significant information, change in patient condition, or equipment problems to the supervisor. Assist in developing and following the teaching plan to educate the patient and family regarding end stage renal disease, dialysis therapy, diet, and medications. Assist with all emergency operational procedures. Administer intravenous medication in accordance with Physician orders and state nurse practice laws. Perform and document pre, interim and post treatment review of patient condition. Initiate basic CPR measures in the event of cardiac and/or pulmonary arrest and respond to emergency situations related to dialysis treatment.

QUALIFICATIONS:
Completion of a practical nursing program approved by the State Board of Nursing. Licensed in the State of Georgia. Current CPR certification. Other certifications may be required based on specific unit guidelines. Successful completion of a physical assessment course. Additional competencies may be required based on specific unit guidelines (ex: ACLS or Action Stat).

Description
An LPN provides support for team and provider(s) during facility encounters.They manage patient traffic flow and perform follow-up care following provider instructions. An LPN also assists with patient education and documents clinical information into the medical record.
Essential Functions:
-Pre-assesses patient by reviewing history, collecting current information and appropriate vital signs, documents in the medical record according to guidelines (e.g., hearing, vision, blood pressure)
-Prepares room and patient for practitioner encounter
-Provides intervention ordered by practitioner (e.g., medication, dressings, specimen collections, irrigations, suture removals, and IVs) and documents patient response
-Initiates interventions and health education according to prevention guidelines
-Educates patient and provides preparation materials for diagnostic studies according to guidelines
-Schedules appointments for patients with lab, radiology, consultants and follow-up appointments as ordered by practitioner.
-Effectively provides prevention, early intervention and ongoing health education for panel, patient and family
-Resolves patient concerns promptly
-Participates in process of interviewing, selection, orientation and training of new team members
-Provides emergency care as necessary and as directed by physician
-Maintains department Quality Improvement standards

Graduate from accredited LPN program with current GA LPN license OR Graduate from an accredited Registered Nurse program with current GA RN license, with at least 3 years experience in a fast-paced Internal Medicine practice w/significant Telephone Triage responsibilities to assist busy practice, located on the main campus of Piedmont Hospital. Performs a variety of patient communication activities in accordance with established standards and practices. Primary focus will be phone triage; may also require call-backs to patients, pharmacies and providing information on lab results. Triages incoming telephone calls; responds to questions regarding medications and treatment instruction; coordinates telephone requests to refill prescription drug medications in accordance with physician’s directions and protocols. Records and reports patient condition; advises physician of any patient adverse reactions to drugs and/or treatments. Provides patient education and teaching; answers questions regarding diagnostic testing and treatment. Must have excellent customer service, interpersonal, and oral/written communication skills and professional demeanor. Clear speaking voice. Must be a team player and pay attention to details. Must be computer literate. Familiarity with NextGen and EMR a plus.
