Field Nurse Case Manager (RN) – Work from Home!
We are currently seeking a Field Nurse Case Manager (RN) for the Metro Atlanta area. Catastrophic Certification from the State Board helpful but not required. Must be a Registered Rehabilitation Supplier with the State Board of Workers’ Compensation (this requires holding a national certification such as CCM and/or CRRN.)
RESPONSIBILITIES:
Responsible for assessment, planning, coordination, implementation and evaluation of injured/disabled individuals involved in the medical case management process. Working as an intermediary between carriers, attorneys, medical care providers, employers and employees, you will closely monitor the progress of the injured worker and report results back to the employer and insurance carrier. This will ensure appropriate and cost-effective healthcare services leading to a medically rehabilitated individual who is ready to return to an optimal level of work and functioning.

Job Description:
The Hospice Referral Coordinator, in conjunction with our Business Development Team, represents VNHS-Hospice Atlanta in the marketplace. This position is responsible for communicating benefits of Hospice Services to targeted audiences as well as to patients and families. Assesses patients for hospice appropriateness, evaluates atients clinical status and gathers necessary information to initiate the admission. Provides a written assessment of the patients needs for hospice services, as well as other required documentation. Responsible for tracking pending patients and assessing patients in a home environment. Serves as a liaison for Visiting Nurse | Hospice Atlanta to facility staff, physicians, patients, families and other organizations.
