SOCIAL WORKER
The incumbent is assigned to the Community Nursing Home and Community Residential Care programs. The social worker provides case management and oversight services to veterans enrolled in both programs under the supervision of the Coordinator of the Community Nursing Home, Community Residential Care Home, Contract Adult Day Health Care and Senior Companion programs.
The incumbent is responsible for providing clinical case management and oversight services to veterans enrolled in the Community Nursing Home and Community Residential Care programs by conducting unannounced alternating monthly visits with the CNH nurse to the contract community nursing homes. The incumbent must document the clinical assessment of the veteran during the visit to the nursing homes in the veteran’s VA medical record.
The incumbent works collaboratively with the VA primary care provider, clinical social worker, and nurse and specialty clinic providers, if applicable to ensure the veteran’s care needs are addressed and the veteran maintains a stable health status in the community.
The incumbent has advanced knowledge of and skill in theory and treatment modalities utilized in treating the geriatric population with chronic physical, psychological and neurological illnesses. The incumbent attends weekly CNH and CRC clinical oversight meetings for veterans assigned to the Community Nursing Home and Community Residential Care programs to discuss veteran’s care plans, psychosocial issues, physical and psychological symptoms which require intervention with members of the CNH/CRC Team.

The Hospice Social Worker is the member of the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient / family unit, define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.
