The incumbent serves as a staff social worker in the Housing Urban Development/VA Supported Housing Program (HUD/VASH) within the Mental Health & Geriatrics Service Line. -Implements plans in a manner that will result in a timely discharge from the hospital
-Works cooperatively with community agencies, assuring that information needed for continuity of care is provided in accordance with hospital policy on confidentiality
-Completes social histories on selected cases, as needed
-Provides short-term counseling to selected patients and families to help them deal with major lifestyle changes or life threatening illnesses
-Documents initial assessment/ plan by next working day after receiving referral. Documentation clearly reflects the initial assessment and the plan for intervention
-Documents all significant contacts in a clear, concise, timely manner

Social Worker (Part-Time)
This position will provide psychosocial services to all patients treated by the facility including in-center and home dialysis patients (if applicable). In this position, you will evaluate each patient at admission and formulate appropriate treatment plan, provide ongoing casework and advocacy services for the patient to ameliorate medically related social or emotional problems, inform and clarify for patients and their families the facility policies and resources. The Social Worker re-evaluates at appropriate intervals those patients not seen on a regular basis as well as maintaining records in accordance with the policies of the Social Service Department and the facility in order to comply with E.S.R.D. regulations.
Master’s degree in Social Work, with a concentration in clinical practice from a CSWE accredited school. Current license: LMSW. Skills: Responsible for psychosocial assessments with patients; conduct assessment and development of plan of care; responsible for responses to crisis situations; serve as liaison between patients and facility staff + other service providers and social agencies; coordination of community events and extensive community outreach for acute medical population.
