Health Coach
PRINCIPAL RESPONSIBILITIES
-Engages program participants in modifying high-risk lifestyle behaviors (e.g., overweight, high stress, low activity level, smoking).
-Assists program participants in developing and/or maintaining self-care plans through utilization of goal setting strategies.
-Applies self-management education principles to changing and maintaining lifestyle behaviors, as indicated by a variety of behavior change principles.
-Uses clinical judgment to make appropriate referrals for program participants.
-Assesses specific education needs.
-Identifies, educates, facilitates and evaluates attainment of self-management goals.
-Provides education about the impact of existing mental health or medical conditions.
-Completes proper documentation of all coaching encounters.
