Outreach Coordinator for Mental Health
Responsibilities:
-Visits providers to introduce disease management and supports/services provided by the program
-Distribute tools and evidence-based medicine materials to providers/physicians in order to assist with quality improvement efforts;
-Respond to physician/provider inquires related to the program; and ensure physicians are integral components of each member’s care management plan;
-Provide provider education in consultation with Medical Director and outside consultants; and
-Participate in community events that promote health and wellness and introduce APS’ disease management program.
-This position requires travel throughout Georgia.
