Nurse, Palliative Care Program
DESCRIPTION:
GENERAL SUMMARY
Responsible for daily program activities, patient coordination and participation as an interdisciplinary member of the Inpatient Palliative Care Team. The activities will include case finding, triaging, assessing, planning, teaching and coordinating care as necessary to respond to the patient’s needs for palliative care.
