Palliative Care Registered Nurse
Job Description:
The Palliative Care RN directs the provision of all Visiting Nurse Hospice Atlanta services necessary to meet the home hospice needs of patients assigned to the care team. This position is responsible for insuring the timely and comprehensive assessment of the patients
needs; the development of individualized care management plans; implementation of
the care plan through the delivery and coordination of services; and the monitoring
and evaluation of all clinical outcomes to insure that the services are provided in a
professional, comprehensive and cost-effective manner.
