Social Worker (LMSW or LCSW) - Healthfield Hospice
As a Social Worker, you will:
-Responsible for making contact with patient, family, and caregiver (p\f\c) within 48 hours of admission.
-Provides skilled assessment of p\f\c psychosocial response to terminal illness and evaluates coping skills in crisis situations within five working days of admission.
-Psychosocial must become part of patient’s chart within 24 hours of completion.
-Assists p\f\c and agency staff in utilizing community resources.
-Assists in educating the p\f\c on grief and loss issues related to the terminal illness, coping skills, and community resources available to meet their needs.
-Maintains accurate on-going documentation of support referrals and the p\f\c responses to support referrals.
-Participates in weekly interdisciplinary team meetings.
