A Social Worker 2 ensures that high quality, comprehensive social work services are provided which support health care and coordinate the psychosocial needs of patients and their families.
ESSENTIAL DUTIES AND RESPONSIBILITIES
-Assists patients and their families in coping with illness and hospitalization based on principles of growth and development.
-Collaborates with hospital personnel and physicians in identifying and managing patients’ physical, social and emotional issues.
-Assists families, hospital personnel and physicians in making plans for discharge related to psychosocial needs.
-Provides information and clarification regarding child protection, custody and safety issues.
-Documents and maintains updated patient information pertaining to services provided.
